How Plan For Health Works

Enrolling in your own annual preventive membership program during your next hygiene visit. Afterwards, you will receive all the preventive benefits that your plan offers, including 15% off all restorative procedures and 10% off all dental products purchased in our office.

The benefits of this plan will only work for services that are received in our office.

The membership fee is payable in full at the time of enrollment.
